Haemostasis in Dentistry

A major consideration with oral surgery is haemostasis.

The mouth is rich in surface and deeper vasculature and some dental procedures can cause difficult-to-control bleeding. Dentists must be prepared for this and have the appropriate haemostatic agents on hand immediately if needed to control bleeding during oral surgery.

What is Haemostasis?

Haemostasis is a complex physiological process that keeps blood inside the blood vessels. If the blood vessel wall is damaged, the vascular wall lining, blood platelets, and coagulation processes collaborate to create a haemostatic plug to prevent bleeding.

  1. Primary haemostasis involves the constriction of the vascular wall as the platelets converge on the area and adhere adjacent to the injury.

 

  1. Secondary haemostasis involves the interaction of products secreted by the platelets and their interaction with fibrinogen to create an insoluble plug or clot which seals the injury to prevent bleeding and facilitate healing.

When a blood vessel ruptures and bleeds uncontrollably, this is referred to as a haemorrhage.

Oral Bleeding in Dentistry

Oral bleeding that is mild is very common in the dental setting, however, it can progress to a haemorrhage quickly in certain circumstances. Controlling bleeding is imperative for the patient’s safety as, otherwise, a medical emergency may ensue.

Before oral surgery, dental extractions, and other such procedures, the dentist must comprehensively assess the patient in terms of their medical history, family history, presence of bleeding disorders or chronic liver disease, and the regular or recent use of drugs such as aspirin, NSAIDs, and blood thinners, as well as the use of Fish Oil, consumption of alcohol, etc.

Control of bleeding in oral surgery and dentistry is affected in numerous ways which may include:

  1. Prolonged, intense compression with a dressing
  2. Application of ice
  3. Electrocautery
  4. CO2 laser
  5. Astringent-styptics (ferric sulphate, tannic acid, aluminium chloride) that mechanically block the haemorrhage
  6. Absorbable gels in sponge, powder, or sheet form – these enable platelet aggregation and stabilize the clot
  7. Collagen sponges (for mild to moderate bleeding)
